New Patient Paperwork at McKinley Chiropractic

At McKinley Chiropractic, our patients have the two options to choose from for completing the necessary New Patient Paperwork for their first appointment.

• You can download and fill out your New Patient Forms from our website. Take a few minutes to enter the requested information, and press the Submit button at the bottom of the forms to submit the paperwork to our office. Feel free to call us with any questions. Please be sure to download the paperwork file to your computer, and open the paperwork file in Adobe Reader in order to correctly submit the forms to our office.
• If you’d prefer to wait until the day of your appointment, that will be great! We ask that you arrive 15 minutes early to allow enough time to complete the intake information before meeting Dr. McKinley.  All of our intake information is completed on an iPad.  The staff will be glad to help you.
